If a campus is submitting multiple new requests in a funding cycle, can the campus a single bank affidavit verifying deposit of non-State funds for all slots requested, rather than an individual affidavit for each request?

Yes. In such cases, the original should be forwarded with the annual submission packet, and each individual request should have a copy attached. Notarized copies are not required if the original is provided.
